Rules:
- Please fill in all the required fields. (Any field that doesn't say (optional) next to it is required.)
- Please allow up to five (5) weeks for your star to be added to the site before submitting a new form for that star.
- We are not a talent agency. Please do not try to add yourself to this site in hope of finding work. If you want to be famous, do an Internet search or look in your local phone book for a reputable talent agency in your area.
- If the star was born after 1997, please do not use this form. Instead, follow the directions on our future teens page.
- Do not try to add stars age 20 years or older, or stars who are about to turn 20.
- Please include information you know to be correct to the best of your knowledge. If you don't know a person's birth name or nickname, do not make them up. Birth dates can usually be found in Google searches, but you may have to estimate from an article about your star if it's not listed anywhere. If you don't know your person's height, you may guess.
- Stars will not be added if they do not have an IMDb (actor/actress) or Wikipedia (other star types) page. Actors and actresses must have at least one credited film role (girl/boy or extra roles do not count, they must have an actual name).
- Requests that do not follow the directions will be ignored/deleted. If you have any questions, feel free to contact the webmiss.
- Teen Star Fan Club reserves the right to edit your submission before it is published.
